ARTIST APPLICATION
Design Proposal Requirements:

Two color copies of each proposed design must be submitted in 8 1/2" x 11" size for approval by the review panel.

Artist must provide a supply list with the total cost of supplies for each design proposal submitted. (The sponsor who selects your design will cover the cost of the listed supplies for chosen design.)

The cost of supplies above the original total given will be the artist's responsibility to cover.
Deadlines:
- Artist applications must be returned for review - November 1, 2007
- Proposed designs submitted for approval - November 1, 2007
